THE OUTREACH PROGRAM PROVIDES PSYCHO-SOCIAL IN-HOME SERVICES TO PATIENTS AND THE FAMILIES OF PATIENTS WITH CHRONIC ILLNESS WHO ARE NOT YET RECEIVING HOSPICE SERVICES. CARE YOUR WAY IS A PRIVATE DUTY PROGRAM THAT PROVIDES HOSPICE SERVICES TO PATIENTS OR FAMILIES WHO MAY REQUEST ADDITIONAL HOURS OF CLINICAL CARE BEYOND THE PRESCRIBED PLAN OF CARE. HOSPICE OF THE VALLEY'S SUPPORTIVE CARE FOR DEMENTIA PROGRAM ASSISTS FAMILIES IN NAVIGATING THE CHALLENGES OF THE DEMENTIA JOURNEY AND SHARES ITS EXPERTISE WITH HEALTH PROFESSIONALS AND FIRST RESPONDERS. THE DEMENTIA CARE AND EDUCATION CAMPUS PROVIDES A FULL SPECTRUM OF CARE OPTIONS FOR PATIENTS AND THEIR CAREGIVERS AT ALL STAGES OF DEMENTIA AND INCLUDES AN EDUCATION CENTER, ASSISTED LIVING FACILITY, AND ADULT DAY CLUB.
