THESE SERVICES EMBRACE A PERSON-CENTERED PHILOSOPHY WHICH EMPHASIZES INDIVIDUALITY, PERSONAL CHOICE, INCLUSION AND INTEGRATION.IN FY 2022 HORIZON PROVIDED SERVICES TO APPROXIMATELY 12,200 BEHAVIORAL HEALTH CLIENTS, 5,300 PRIMARY CARE PATIENTS, AND OVER 1,600 INDIVIDUALS WERE SERVED THROUGH MOBILE CRISIS INTERVENTIONS. HORIZON HOSTS AND PARTICIPATES IN NUMEROUS AWARENESS, HEALTH AND (CONT'D ON SCHEDULE O) WELLNESS EVENTS THROUGHOUT THE YEAR IN THE COMMUNITIES WE SERVE. WE CONDUCT COMMUNITY TRAINING PROGRAMS AND COORDINATE OR PARTICIPATE IN VARIOUS COMMUNITY SUPPORT PROGRAMS.HORIZON PROVIDED COVID19 TESTING, VACCINATIONS, AND ESSENTIAL RESOURCES IN THE EARLY STAGES OF THE PANDEMIC, SPECIFICALLY IN THE ARIZONA COMMUNITIES WE SERVE. USING A CONVENIENT DRIVE UP VACCINATION MODEL AND ONLINE REGISTRATION PROVIDED EASIER ACCESS TO COMMUNITY MEMBERS.THROUGHOUT THE PANDEMIC, OUR EFFORTS HAVE BEEN GUIDED BY OUR INTEGRATED HEALTHCARE APPROACH, ADAPTING TO THE NEEDS OF THE COMMUNITIES, WHICH INCLUDED OFFERING TELEMEDICINE SERVICES. TELEMEDICINE WAS CRITICAL AS WE CONTINUED TO SERVE CHILDREN, ADULTS, AND FAMILIES THROUGHOUT ARIZONA DURING THE PANDEMIC. MANY CHRONIC PATIENTS SCHEDULED TELEMEDICINE SERVICES TO AVOID FACE-TO-FACE CLINIC VISITS, WHICH HELPED TO MINIMIZE THEIR RISKS OF EXPOSURE TO COVID19, WHILE RECEIVING THE CARE THEY NEED.
